Medications for Alcohol Dependency
- Oral Naltrexone and Extended-Release Injectable Naltrexone: These medications are indicated for the treatment of alcohol dependence in patients who can abstain from alcohol in an outpatient setting before beginning treatment.
- Acamprosate: This medication is used for the maintenance of abstinence in patients who are dependent on alcohol and are abstinent at the initiation of treatment.

If you are drinking more than you want to, that is a medical issue with real treatments — and it is a conversation you can have here without judgment. Dr. Rajan Kohli is board-certified in Family Medicine and evaluates and treats alcohol use disorder in the primary care setting. Most visits are covered by insurance.
How much is too much?
Guidelines define at-risk drinking as more than 4 drinks in a day or 14 in a week for men, and more than 3 in a day or 7 in a week for women. But the more useful test is personal: if it is affecting your health, work, relationships or sleep, or you have tried to cut down and could not, it is worth addressing regardless of the number.
Is medication available?
Yes, and it is significantly underused. Naltrexone and acamprosate are FDA-approved for alcohol use disorder and have good evidence for reducing drinking and supporting abstinence. Disulfiram is an option in specific circumstances. These can be prescribed and monitored in primary care.
Do I have to stop completely?
Not necessarily. For some patients the goal is abstinence; for others, reduced drinking is a realistic and worthwhile target, and there is evidence supporting that approach. The goal is set with you rather than imposed on you.
Is it dangerous to stop on my own?
It can be. If you drink heavily and daily, stopping abruptly can cause serious withdrawal including seizures and delirium tremens, which can be life-threatening. Talk to Dr. Kohli before you stop so withdrawal can be managed safely — this is genuinely important.
Is this confidential?
Yes. It is protected health information like any other part of your medical record, with the additional federal protections that apply to substance use treatment records.
