Led by Dr. Rajan Kohli, a board-certified Family Medicine physician, our practice is dedicated to helping patients achieve optimal health and wellness through personalized, patient-centered care.

Rajan Kohli MD

Dr. Kohli completed his Family Medicine residency in Michigan in 2005 and brings more than 20 years of diverse medical experience to his practice. He previously served as Chief of Medicine and Medical Director of the Hospitalist Unit at both Baylor Scott & White Medical Center – Waxahachie and White Rock Medical Center. His professional background includes extensive experience in outpatient medicine, hospital care, nursing home management, newborn nursery care, and emergency medicine.

In addition to his broad clinical background, Dr. Kohli completed a fellowship through A4M in Functional, Integrative, and Regenerative Medicine. He is passionate about combining traditional medicine with holistic and preventive strategies to help patients achieve a healthier, more balanced life while reducing unnecessary medication burden whenever appropriate.

Lakewood Primary Care and Wellness is now located inside the Professional One Building on the Presbyterian Hospital campus. Within the same office, Dr. Kohli also offers Lakewood Med Spa, where patients can access facial aesthetic services and a full range of medically supervised med spa treatments.
