HIV Pre-exposure Prophylaxis (PrEP):
- PrEP is a groundbreaking biomedical advancement that offers effective protection against HIV when taken before potential exposure. The two commonly prescribed PrEP medications are Truvada (tenofovir disoproxil fumarate 300 mg/emtricitabine 200 mg) and Descovy (tenofovir alafenamide 25 mg/emtricitabine 200 mg). Both medications are taken as one pill daily to ensure maximum protection against HIV infection. Studies show that consistent daily use can lower the risk of HIV by as much as 90%. However, its effectiveness significantly decreases if not taken consistently.
- PrEP with Doxycycline for Enhanced Protection: For those at higher risk of STDs, we also recommend considering the use of doxycycline on an as-needed basis in conjunction with PrEP. Doxycycline, taken as post-exposure prophylaxis (PEP) at a dose of 200 mg within 24-72 hours after potential exposure, can help reduce the risk of acquiring bacterial STDs like syphilis, chlamydia, and gonorrhea. This strategy, often referred to as "PrEP 2.0," provides additional protection alongside daily PrEP.
Recommendations:
- Daily PrEP: Take “Truvada” or “Descovy” daily to ensure maximum protection against HIV. Consistent use is key to maintaining its effectiveness.
- PRN Doxycycline: Consider taking “doxycycline 200 mg” as a preventive measure after potential exposure to bacterial STDs. This approach can provide additional protection and peace of mind, especially for those with higher exposure risks.

How quickly can I be tested?
Usually within a few days, and often same-week. If you have had a specific exposure, call and say so — timing matters for which tests are accurate, and some situations need to be seen urgently rather than scheduled.

What is PrEP and am I a candidate?
PrEP is a daily or long-acting medication that substantially reduces the risk of acquiring HIV. Candidacy depends on your risk factors and requires baseline HIV testing, kidney function testing and hepatitis B status before starting, then follow-up testing on a regular schedule while you remain on it.
Is PrEP covered by insurance?
Most commercial plans cover PrEP and the associated monitoring, and many cover it without cost sharing. Coverage varies, and assistance programs exist. Our staff verifies your benefits before your appointment and will tell you what to expect.
Do you treat partners as well?
We can see partners as patients. Depending on the infection and Texas requirements, expedited partner therapy may be an option, and Dr. Kohli will discuss what applies in your situation.
