Thyroid conditions at Lakewood Primary Care and Wellness are evaluated, diagnosed, treated and monitored by Dr. Rajan Kohli, MD, board certified in Family Medicine and fellowship trained in Functional, Integrative and Regenerative Medicine (A4M), in Dallas, Texas. Symptoms that bring patients in

An underactive thyroid (hypothyroidism) is the pattern we see most: persistent fatigue, weight gain that resists effort, feeling cold when others are not, constipation, dry skin, thinning hair, brain fog, low mood, and heavy or irregular periods. An overactive thyroid (hyperthyroidism) tends to run the other way — palpitations, unexplained weight loss, heat intolerance, anxiety, tremor and disrupted sleep. Thyroid disease is several times more common in women, and its symptoms overlap heavily with perimenopause — which is exactly why testing matters rather than guessing.

How we test

Screening usually starts with TSH, but a single TSH value can miss the picture. When symptoms or history warrant it, we order a fuller panel: TSH, free T4, free T3, and thyroid antibodies (TPO and thyroglobulin antibodies) to identify Hashimoto's thyroiditis — the most common cause of hypothyroidism in the United States — and Graves' disease. Because thyroid symptoms are rarely one thing, we also look at the neighbors: blood count, iron stores, vitamin B12 and vitamin D, blood sugar, and — where the timeline fits — sex hormones. If we find a thyroid nodule or an enlarged gland, we arrange an ultrasound and, when indicated, referral for biopsy.

How we treat

Treatment is matched to the diagnosis, not a package. For hypothyroidism the standard of care is levothyroxine, dosed carefully and rechecked about 6–8 weeks after any change until you are stable, then monitored once or twice a year. For patients who continue to have symptoms despite normal labs on levothyroxine, we discuss the evidence honestly — including where combination T4/T3 therapy does and does not have support — and make the decision together. Hyperthyroidism, Graves' disease and thyroid nodules are managed in coordination with endocrinology when specialist care is the right answer; part of good primary care is knowing when to refer.

The functional medicine angle — honestly framed

Dr. Kohli's functional and integrative training means the visit does not stop at a prescription. We review the contributing factors that affect how patients with thyroid disease actually feel — iron and B12 status, sleep, blood sugar, medication interactions (including how and when thyroid medication is taken), and hormonal transitions. What we do not do is replace proven treatment with supplements or promise cures. Care is billed to Medicare or your insurance like a normal primary care visit.

Do I need an endocrinologist, or can a primary care doctor treat my thyroid?

Most hypothyroidism is diagnosed and managed in primary care, and that is what we do daily. Endocrinology referral is the right answer for Graves' disease, suspicious nodules, thyroid cancer follow-up and complex cases, and we coordinate that referral when it is needed.

What thyroid tests will you order?

My TSH is "normal" but I still feel exhausted. Is it worth retesting?

Often, yes. A single TSH can miss early autoimmune thyroid disease and says nothing about iron, B12, sleep, mood, blood sugar or hormonal transitions, all of which produce the same symptoms. We evaluate the whole differential rather than stopping at one number.

What is Hashimoto's thyroiditis?

Hashimoto's is an autoimmune condition in which the immune system gradually damages the thyroid gland. It is the most common cause of hypothyroidism in the United States. It is identified with antibody testing and managed with thyroid hormone replacement and scheduled monitoring.

Will I be on thyroid medication forever?

Usually, if the cause is Hashimoto's, since the gland does not recover. Some causes are temporary, such as thyroiditis after pregnancy or a viral illness, and medication can sometimes be reduced or stopped. We recheck labs about 6 to 8 weeks after any dose change, then once or twice a year.

Do you prescribe T3 or combination thyroid medication?

Levothyroxine (T4) is the standard of care and works well for most patients. For those who remain symptomatic despite normal labs, we discuss the evidence for combination T4/T3 therapy honestly, including its limits, and decide together based on your response and follow-up labs.

Could my symptoms be menopause rather than my thyroid?

They genuinely can be either, or both. Fatigue, weight change, mood shifts and sleep problems belong to both conditions, which is why we test rather than guess. Because we also evaluate and treat hormones for women, both possibilities can be worked up in one place.

Do you accept Medicare and insurance for thyroid care?
