Sexual Health: Common Questions

Sexual health concerns — low libido, erectile dysfunction, pain with intercourse, or changes after menopause — are common medical issues with medical causes worth evaluating. Dr. Kohli is board-certified in Family Medicine and evaluates these concerns privately, as part of ordinary primary care. Many visits are covered by insurance.

Is low libido a medical problem?

It can be. Reduced sex drive is often driven by something identifiable — thyroid disease, low testosterone, anemia, diabetes, depression or anxiety, sleep disruption, relationship stress, or a side effect of a medication such as an antidepressant or blood pressure drug. Evaluation starts with history and targeted lab work rather than assuming a single cause.

What causes erectile dysfunction?

Erectile dysfunction is frequently an early sign of vascular disease, which is why it is worth evaluating rather than only treating. Common contributors include high blood pressure, diabetes, high cholesterol, smoking, obesity, low testosterone, medication side effects, alcohol, sleep apnea, depression and anxiety. In younger men it is more often psychological, but it is still assessed medically.

What does an evaluation involve?

A confidential history, a focused physical examination, and laboratory testing chosen for your situation — which may include testosterone, thyroid function, blood glucose or A1c, cholesterol, and a blood count. The purpose is to identify treatable medical contributors and to screen for cardiovascular risk that may not have surfaced yet.

Is this covered by insurance?

The evaluation visit and diagnostic laboratory testing are generally covered when medically necessary, the same as any other primary care problem. Coverage for specific treatments varies considerably by plan, and some are not covered. Our staff verifies your benefits before your appointment and will tell you what to expect.

Are these conversations confidential?

Yes. Everything discussed is protected health information under HIPAA and is part of your confidential medical record, the same as any other visit. Many patients find it easier to raise these concerns with their primary care physician, who already knows their history and medications, than to start again with someone new.

Can painful intercourse be treated?

Yes, and it should be evaluated rather than tolerated. Pain with intercourse has identifiable causes including vaginal dryness and tissue changes after menopause, infection, skin conditions, pelvic floor muscle dysfunction and endometriosis. Treatment depends entirely on the cause, so evaluation comes first.
