What is hormone pellet therapy?

A pellet is a small implant placed under the skin, usually in the upper hip, during a brief in-office visit. It releases hormone steadily over several months rather than requiring a daily gel, patch or weekly injection. For many patients that consistency is the appeal — there is nothing to remember, nothing to apply, and no daily peaks and troughs.

What happens at the insertion appointment?

The visit takes about fifteen minutes. The area is cleaned and numbed with local anesthetic, a small incision is made, the pellet is placed, and the site is closed with adhesive strips. Most patients describe pressure rather than pain. You go back to your day afterward, avoiding swimming, hot tubs and heavy lower-body exercise for a few days while the site heals.

How long does a pellet last?

Typically three to four months for women and four to six months for men, though it varies with your dose, metabolism and activity level. Rather than guessing, Dr. Kohli repeats your labs and reviews your symptoms to determine when you are due, so timing is based on your levels rather than a fixed calendar.

Why have a physician do this rather than a med spa?

Because hormone therapy is medicine, not a service. It requires a diagnosis before treatment, interpretation of labs in the context of your full medical history, attention to your other prescriptions, and ongoing monitoring of the things that need watching — blood count and prostate markers in men, and continued age-appropriate screening in women. Dr. Kohli evaluates, places and monitors, and he is the same physician managing the rest of your health.

What testing do you do first?

Treatment follows testing, never the reverse. Depending on your symptoms and sex, that usually means sex hormone levels, thyroid function, a complete blood count and metabolic testing. Testosterone is measured in the morning and confirmed on a second sample before any diagnosis of deficiency is made. Symptoms like fatigue and low libido overlap with thyroid disease, anemia, sleep apnea and depression, and those are evaluated too.

What are the risks, and how are they monitored?

Risks depend on the hormone, dose, your age and your history, and are discussed with you individually before you start. Site effects such as bruising, tenderness or, uncommonly, pellet extrusion or infection can occur. Testosterone therapy can raise red blood cell count and affect fertility and prostate markers. A pellet also cannot be adjusted or removed as easily as an oral or topical dose once placed. Dr. Kohli monitors with scheduled labs and adjusts your dose over time.

Is hormone therapy covered by insurance?

The evaluation visit and laboratory testing are generally covered when medically necessary, and we verify your benefits before your appointment. Pellet insertion itself is most often a self-pay service. Dr. Kohli will give you the full cost — pellets, placement and follow-up labs — before you commit to anything.
