Dr. Rajan Kohli provides thoughtful, unhurried primary care for older adults through convenient office visits and telehealth. We help families keep a parent’s care organized — medications, chronic conditions, memory concerns, and follow-up after a hospital stay — with a physician who listens and helps you stay ahead of problems.
Care is provided at our North Dallas office and by telehealth. We do not provide routine on-site rounding inside assisted-living facilities.
When a parent moves into assisted living, medical care can become fragmented across specialists, a hospital, a pharmacy, and the community’s staff. Our role is to be the steady center of that care: one physician who knows your parent, keeps the medication list straight, watches the chronic conditions, and helps you know what to do when something changes. With your parent’s permission, we keep you in the loop so you are not piecing it together alone.
Transportation to the office is arranged by the family, community, or a transportation service. Telehealth works best when someone can help your parent with the device if needed.
A full reconciliation of prescriptions, over-the-counter drugs, and supplements — checking for duplicates, interactions, and medicines that can raise the risk of falls or confusion. Where it’s clinically appropriate, we look for what can be simplified or safely reduced.
Steady, attentive care for diabetes, high blood pressure, heart and lung conditions, thyroid disease, kidney disease, arthritis, osteoporosis, and mood — with clear goals, lab follow-up, and coordination with specialists.
A careful look at new confusion or memory changes — considering medications, sleep, mood, hearing and vision, and whether further testing or a referral is needed. We won’t rush to conclusions or promise a diagnosis in one visit.
One of the most important visits there is. After a hospital or emergency visit we review the discharge plan, reconcile medications, confirm follow-ups, and watch for warning signs — the window when problems most often slip through.
We work with your parent’s specialists, pharmacy, home-health team, and the assisted-living staff — and, with authorization, with you — so everyone is working from the same plan.
A personalized prevention visit: fall-risk and safety review, immunization and screening review, cognitive and mood screening when appropriate, and advance-care-planning conversations on your family’s terms.

An adult child can arrange the appointment, and we welcome your involvement. Because your parent’s health information is protected, detailed discussions may require your parent’s permission or a valid healthcare power of attorney — please bring that documentation if it applies. We’ll always tell you what we can share and how to be included properly.
Insurance: We accept Medicare and participate with select Medicare Advantage plans. Network participation and referral rules vary by plan — please call us with your parent’s exact plan name and we’ll verify we’re in-network before the first visit.

Yes. We welcome new Medicare patients and patients on the Medicare Advantage plans we participate in, and we become the ongoing primary care physician — managing chronic conditions, prevention, medications, and follow-up over time.
With your parent’s permission or a valid healthcare power of attorney, we keep you informed and included. Bring any proxy or power-of-attorney paperwork and we’ll explain what we can share.
Yes — this is one of the most valuable visits. We review the discharge plan, reconcile medications, confirm follow-up, and watch for warning signs. Call us and we’ll help arrange the earliest appropriate appointment.
We participate with select Medicare Advantage plans, and rules vary by plan. Call us with the exact plan name or member ID and we’ll confirm we’re in-network before scheduling.
