What Medicare covers at a primary care visit
Medicare covers the primary care most older adults need: management of chronic conditions such as diabetes and high blood pressure, preventive screenings, the yearly wellness visit, and coordination of your medications and specialists. Standard office visits for illness and follow-up are covered as well, subject to your plan's cost-sharing.
The Annual Wellness Visit, explained
The Medicare Annual Wellness Visit is a yearly, preventive planning visit — not the same as a head-to-toe physical. It reviews your health history, screens for risks and cognitive changes, updates your prevention schedule and builds a personalized plan for the year. Medicare Advantage or Original Medicare — both are welcome here
We care for patients on Original Medicare (often with a supplement) and on Medicare Advantage plans from all major carriers. The right choice for you depends on your health, your other coverage and your budget, and we are happy to see you on either. We do not steer patients toward any particular plan; bring the coverage you have and we will work with it.
Chronic Care Management for Medicare patients
If you live with two or more ongoing conditions, Medicare's Chronic Care Management benefit supports coordinated care between visits — a documented care plan, medication oversight and a point of contact for questions. It pairs naturally with our chronic disease care and helps keep small problems from becoming hospital stays.
Turning 65 — what to do first
As you approach 65, establishing care with a primary care physician early makes everything smoother: a baseline visit, your medications reviewed, screenings scheduled, and a familiar office when something comes up. You do not need to wait until a problem appears — call and we will help you get started.

Preventive services such as the Annual Wellness Visit are generally covered at no cost under Original Medicare; other visits follow your plan's deductible and cost-sharing. Prefer more time and direct access to your physician? An optional concierge membership is available as an add-on for patients who want it — it is never required, and your Medicare coverage is used for your medical care either way.
