Geriatric care is primary care built around the realities of getting older — multiple conditions, long medication lists, memory and balance concerns, and coordination between several specialists. Dr. Rajan Kohli is board-certified in Family Medicine and cares for a large Medicare panel. Most visits are covered by Medicare.
What makes geriatric care different from regular primary care?
Time and coordination. Older patients often have several conditions interacting with each other and a medication list where one drug treats the side effect of another. Geriatric care means reviewing the whole list at each visit, watching for interactions, and prioritising what matters most to you rather than treating each number in isolation.
Does Medicare cover these visits?
Yes. Medicare Part B covers medically necessary visits, and Medicare covers an Annual Wellness Visit each year at no cost to you. Some patients also qualify for Chronic Care Management between visits. Coverage and cost share vary by plan, and our staff verifies your benefits beforehand.
Can you review my medications and reduce them if possible?
Yes, and it is one of the most valuable things we do. Bring every bottle, including supplements and anything prescribed by a specialist. Dr. Kohli looks for duplicates, drugs no longer needed, doses that should change with kidney function or age, and combinations that raise fall risk. Sometimes the right answer is stopping a medication rather than adding one.
Do you evaluate memory concerns and fall risk?
Yes. Memory changes are evaluated rather than assumed to be normal ageing — thyroid disease, B12 deficiency, depression, sleep apnea and medication effects can all contribute and are treatable. Balance, gait, vision, blood pressure on standing and home hazards are reviewed for anyone at risk of falls.
Can family members come to appointments?
Yes, and we encourage it for patients who want that support. With your permission we can speak with a family member or caregiver about your care. Bringing someone who helps manage medications at home often makes the visit far more useful.
