Chronic Care at Lakewood: Common Questions
Chronic care is the ongoing medical management of conditions like high blood pressure, type 2 diabetes, high cholesterol, thyroid disease and heart disease. Dr. Kohli is board-certified in Family Medicine and manages these conditions long term — regular visits, lab monitoring, medication adjustment, and coordination with your specialists. Most chronic care visits are covered by Medicare and commercial insurance.
What is chronic disease management?
Chronic disease management is regular, planned medical care for a condition that does not go away — such as hypertension, type 2 diabetes, high cholesterol, thyroid disease, COPD or heart disease. It means scheduled visits rather than only sick visits, lab work reviewed on a set interval, medication adjusted as your numbers change, and one physician keeping track of the whole picture.
How often should I be seen for diabetes or high blood pressure?
Most patients with stable hypertension or type 2 diabetes are seen every three to six months, with lab work before the visit. If your blood pressure or A1c is not yet at goal, or a medication has just changed, visits are usually closer together until things settle. Dr. Kohli sets the interval with you at each visit.
Does Medicare cover chronic care visits?
Yes. Medicare Part B covers medically necessary visits for chronic conditions, and Medicare also covers an Annual Wellness Visit each year. Some patients additionally qualify for Chronic Care Management services between visits. Coverage and any cost share depend on your specific plan, and our staff verifies your benefits before your appointment.
Can you manage my conditions alongside my cardiologist or endocrinologist?
Yes, and that is usually how it works. Dr. Kohli manages your day-to-day medical care and coordinates with your specialists — sharing labs, avoiding duplicate testing, and watching for interactions between medications prescribed by different physicians. Having one physician who sees the entire medication list matters most for patients taking several.
What should I bring to a chronic care visit?
Bring all current medications and supplements in their bottles, or a written list with doses; any home blood pressure or blood sugar readings; lab work or hospital records from the past year, including anything you were told was normal; and your insurance card. Home readings are genuinely useful — a single reading in the office is a snapshot, not the pattern.
What happens if my numbers are not improving?
We look at the whole picture before adding another medication: whether the current dose is right, whether the medication is being taken as prescribed and tolerated, cost or access barriers, and contributing medical, behavioral and environmental factors such as sleep, diet, alcohol, activity and stress. Sometimes the answer is a different medication; sometimes it is removing one.
