Under the leadership of Dr. Rajan Kohli, a board-certified Family Medicine physician, our practice is committed to guiding our patients toward optimal health and well-being.
Dr. Kohli completed his Family Medicine residency in Michigan in 2005 and brings over 20 years of diverse medical experience to our practice. Dr. Kohli is currently on staff at Texas Health Presbyterian Hospital Dallas and previously served as Chief of Medicine at White Rock Medical Center and at Baylor Scott & White Waxahachie. He has extensive experience in office practice, hospital care, nursing home management, newborn nursery care, and emergency medicine.
In addition to his extensive clinical background, Dr. Kohli has completed a fellowship in Functional, Integrative, and Regenerative Medicine through A4M. He is passionate about integrating these holistic approaches with traditional medical care to help patients achieve a balanced and healthy life.

We care for patients aged three and older, from adolescent and pediatric care through adult primary care and geriatric and senior care.
Functional and integrative medicine looks for the root cause of symptoms rather than treating them in isolation. At Lakewood we combine conventional primary care with a whole-person approach across eight areas: mind, lifestyle, gut health and detoxification, inflammation and immune balance, anti-aging, hormone and neurotransmitter balancing, stress management and medication.
We offer an optional concierge membership for patients who want enhanced access, but it is entirely optional. We continue to accept and care for Medicare, insurance and self-pay patients without any membership.
Yes. We provide physician-supervised weight loss with personalized plans that may combine prescription medication, including GLP-1 options, with nutrition, stress, sleep and exercise strategies. Your plan is tailored to your health history during a consultation.
