How do I book an appointment?
Call 214-810-4331, or request an appointment online. Same-week appointments are usually available, and no referral is needed for primary care.
What are your hours?
Monday through Friday, 8:00 AM to 5:00 PM. Telemedicine visits are available for established situations that do not need an in-person exam.
Are you accepting new patients?
Yes — including new Medicare and Medicare Advantage patients. Most new patients are seen the same or following week.
What insurance do you accept?
Medicare, Medicare Advantage and Medicare Supplement plans, plus most major commercial insurance: Blue Cross Blue Shield of Texas, Aetna, UnitedHealthcare, Cigna, UMR and Baylor Scott & White Health Plan among them. We can verify your specific plan before your visit.
Do you accept Medicare?
Yes. Medicare patients are the core of the practice, particularly adults managing high blood pressure, diabetes and cholesterol. We provide Medicare Annual Wellness Visits and Chronic Care Management, both covered Medicare benefits.
What ages do you see?
Age three and up — children, adults and seniors. Family medicine covers the whole household, and a large share of the practice is adults over 65.
Who is Dr. Rajan Kohli?
Dr. Rajan Kohli, MD is a board-certified Family Medicine physician with more than twenty years of experience. He is on staff at Texas Health Presbyterian Hospital Dallas and previously served as Chief of Medicine at White Rock Medical Center. He is fellowship trained in Functional, Integrative and Regenerative Medicine through A4M.
What is Dr. Kohli board certified in?
Family Medicine. His A4M fellowship in Functional, Integrative and Regenerative Medicine is additional training, not a board certification — we state that plainly because credentials are something patients are entitled to verify.
Who else is on the care team?
Two physician assistants: Allison Barnes, PA-C and Alysa Casey, PA-C. Every patient's care is directed by Dr. Kohli.
Which hospital are you affiliated with?
Texas Health Presbyterian Hospital Dallas — the office is on that hospital's campus, which simplifies imaging, labs and admission when they are needed.
What services do you offer?
Primary and preventive care, annual and sports physicals, chronic disease management, same-day sick visits, pediatric care (age 3+), geriatric care, well woman exams, telemedicine, medical weight loss, hormone replacement therapy for men and women, thyroid care, sexual health, functional and integrative medicine, and optional concierge membership.
Can I be seen the same day if I am sick?
We hold room for acute visits and can usually see sick patients the same or next day. Call 214-810-4331 first thing and we will fit you in or advise you honestly if the ER or urgent care is safer.
Do you offer telemedicine?
Yes. Video visits are available for follow-ups, medication reviews, results discussions and many acute concerns that do not need a hands-on exam. Telemedicine is billed to insurance like a normal visit in most plans.
What is the Medicare Annual Wellness Visit?
A yearly prevention-planning visit covered by Medicare Part B at no cost to you: health risk assessment, medication review, memory and fall-risk screening, and a written prevention plan. It is not a head-to-toe physical — and we explain the difference before anything is billed.
Do you do well woman exams and Pap smears?
Yes. Well woman exams, Pap smears, breast exams, contraception counseling and menopause care are a core, steady part of the practice, billed to insurance as preventive care.
How does your medical weight loss program work?
It is physician-supervised: full evaluation first, then a plan that can include GLP-1 medications such as semaglutide or tirzepatide when clinically appropriate, with insurance benefits verified and a realistic cost estimate before you start, and scheduled follow-up throughout.
Do you offer hormone replacement therapy?
Yes, for both men and women. Every patient is tested before treatment and followed with scheduled labs. Options include pellets, injections, patches, gels and oral routes, chosen with Dr. Kohli based on your labs, symptoms and preferences.
Do you provide geriatric care?
Yes. Care for adults over 65 — medication review, memory and fall-risk evaluation, chronic disease management and coordination with specialists — is central to the practice, and Medicare is accepted.
Is concierge membership required to be a patient?
No. Standard insurance-billed care is the foundation of the practice and always will be. Concierge membership is an optional add-on for patients who want extended visits and direct physician access — it is never required.
What is your functional and integrative medicine approach?
It means the visit does not stop at a prescription: we evaluate contributing medical, behavioral and environmental factors — labs, sleep, nutrition, medications, hormones — alongside conventional treatment. It is billed to Medicare or insurance like normal primary care.
Can a family medicine doctor manage complex adult conditions, or do I need an internist?
Either specialty is appropriate for adult chronic disease — both are primary care. Dr. Kohli is board certified in Family Medicine, with twenty-plus years managing hypertension, diabetes and the conditions internists manage, plus care for the rest of the household.
Do I need a referral to be seen?
No. Primary care is the front door of medicine — you can book directly. If your insurance requires you to name a primary care physician, we can be that PCP.
